[BUG] claude code Ultrareview failed

What's Wrong?

Minutes after executing the /ultrareview command, it fails and does not recover, resulting in loss of credit and no resolution.

What Should Happen?

The execution of the /ultrareview command should complete without errors and generate the report.

It simply fails without any error message.
