[Bug] Claude Code not retaining task list items and skipping tasks during execution

Bug Description
absolutly horrible today. it keeps coding when i tell it now to at all. We have a list built for thing that need to be done and when i ask for the calude full list to match mine it refuses to give it to me. I had to make a side list beacseu claude is not retaining the item that need to be done and jsut skips stuff so i constantly compare my list to caldue to be sure we are on the same page. 9/10 claude leaves stuff out
