[BUG] [BUG] Create PR notification above query line in Claude.app Code window shows changes to untracked files in counts.

What's Wrong?

The "Create PR" notification appears above the query line when the only modified files are Claude environment files that are not tracked and are in untracked/gitignored directories.

What Should Happen?

Changes to untracked files, which do not require a pull request, should not show the Create PR prompt.

Steps to Reproduce

1. Make changes to files in .claude inside the current repo.
2. Observe the Create PR notification appear.
