Anthropic API Usage Policy Violation Warnings During Documentation Generation

Bug Description
I keep receiving these messages that I am violating the usage policy. I am just using the model to generate documentation and planning documents with structured prompts provided by an MCP . why do I get this messages and how can I fix it
