[Bug] Prompt cache not being utilized, causing excessive token consumption on simple requests

Bug Description
The prompt cache in Claude Code is completely broken. Every message is processed from scratch without reading from cache, consuming 16,000–26,000 tokens on a simple "hello" in an empty folder with no special configuration.
