[BUG] [VSCode] Chat panel: "Accept Plan" button unreachable when many comments are added in Plan View – cannot scroll to submit

What's Wrong?

When reviewing a plan and adding a significant amount of feedback comments in the Plan View, those comments are reflected in the "Accept Plan" dialog in the chat panel. However, when many comments have been added, the "Accept Plan" / submit button at the bottom of the dialog becomes invisible and unreachable — the chat panel does not allow scrolling far enough to reach it.
Steps to Reproduce

Start a task that triggers plan mode in the VS Code extension.
In the Plan View panel, add detailed feedback/comments to multiple plan steps — enough content that the resulting "Accept Plan" dialog in the chat panel exceeds the visible area.
Switch to the chat panel where the "Accept Plan" dialog is shown.
Try to scroll down to the submit / "Accept Plan" button at the bottom of the dialog.

What Should Happen?

The chat panel should be fully scrollable, ensuring the "Accept Plan" button is always accessible regardless of how many comments have been added in the Plan View.
