[BUG] Claude Code edits my files without asking, despite "Ask Before Edits" being on

What's Wrong?

On the latest version of Claude Code (and the previous ~5 versions), Claude edits files without asking. I always have "Ask Before Edits" enabled. When I confront Claude and say, "why aren't you showing me diffs in my IDE before editing files" it shows me diffs within the chat interface, but still edits the files directly without stopping to ask.

This has been more than two weeks. I've reset all settings. I've logged out and back in, uninstalled and reinstalled.

I am using Claude Code via the VSCode interface, using latest VSCode and Claude (updated both today)

What Should Happen?

Claude should ask before editing files when "Ask Before Edits" is enabled.

I am not sure - this problem isn't happening for anyone else on my team. I've cleared ~/.claude.json, ~/.claude, logged out and back in, uninstalled and reinstalled.
