[BUG] [VSCODE] Fails to re-prompt (ask user tool) when opening a previous session after a restart

What's Wrong?

When I open a previous session after reloading VS Code, if the agent stopped at the ask user tool, it doesn't restore the prompt.

What Should Happen?

Re-show the prompt when I open the session

Steps to Reproduce

1. Start a session (plan or something to get a prompt)
2. Restart VS Code
3. Re-open the session and observe that there is no re-prompt
