[BUG] [VSCODE] Fails to re-open plan when opening a previous session after a restart (or have any way to re-open the plan manually)

What's Wrong?

When I open a previous session after reloading VS Code, if the agent stopped at the asking about a plan, the plan isn't re-shown, can't even click on the "Claude Plan" entry in the chat

What Should Happen?

Re-show the plan

Steps to Reproduce

  1. Start a planning session and wait for the plan to show up/wait for user
  2. Restart VS Code
  3. Re-open the session and observe that there is no plan opened, nor can you click on the Claude Plan item in the chat
