What's Wrong?

When I open a previous session after reloading VS Code, if the agent stopped at the asking about a plan, the plan isn't re-shown, can't even click on the "Claude Plan" entry in the chat
What Should Happen?
Re-show the plan

Steps to Reproduce
- Start a planning session and wait for the plan to show up/wait for user
- Restart VS Code
- Re-open the session and observe that there is no plan opened, nor can you click on the Claude Plan item in the chat
