Bug in Ask before editing

There is a really annoying bug in the most recent update of claude in VSCode. I often use "Ask before editing" and when I'm reviewing a change claude just made and writing my feedback in the chat window, it somehow seems to 1/ get me out of editing 2/ what I've written is lost 3/ the change is auto-accepted and 4/ the agent switches to "edit automatically". This has happened over 10 times in the last couple days despite me being very careful - and is really annoying - once claude accepts and moves on, I've lost the ability to review the changes (unless I do that manually). Very disruptive to the workflow. Please fix ASAP.
