[BUG] Switching to accept edits mode mid-session in a dynamic workflow keeps asking for permission.

What's Wrong?

When a subagent in a dynamic workflow makes a file edit and you choose "Yes, allow all edits this session" in the permission dialog, the session changes to accept edits mode, but this setting doesn't get applied to any of the subagents already running, even the one for which you chose the option.

What Should Happen?

Claude Code should stop presenting a write/edit permission dialog when the mode has been switched to accept-edits.

1. Start a dynamic workflow with manual approval required for edits
2. Choose "Yes, allow all edits this session" on the first permission prompt
