[Bug] Claude Code ignoring instructions from memory/Claude.md files and hallucinating solutions

Bug Description
Claude Code is not following instructions linked in memory or Claude.md. Claude is consistantly hallucinating and making up soultions that are clearly not correct. Claude Code model Opus 4.6 (1M context) default auto. Using in Claude Code. This isssue seems to be getting worse every day.
