[Bug] Claude ignoring user instructions in Claude Code CLI

Bug Description
Claude Opus 4.8 high is not following direct instructions to do a real user test on a product because it is "heavy". It instead opts for a simulated workaround for expediency and proceeds without pausing for confirmation until stopped manually.
