[Bug] Voice mode spacebar shortcut unresponsive until manually toggled after startup

Bug Description
Title: voiceEnabled: true in settings.json does not activate voice mode on startup — requires manual toggle to initialize

Description:

When voiceEnabled: true is set in ~/.claude/settings.json, voice mode appears visually enabled on startup but the spacebar shortcut does not work until the user manually toggles voice mode off
and back on within the session.

Steps to reproduce:

1. Set "voiceEnabled": true in ~/.claude/settings.json
2. Open a new terminal and start a Claude Code session
3. Press spacebar to activate voice input
4. Nothing happens

Expected behavior:
Voice mode is fully active on startup — spacebar triggers voice input immediately.
