[BUG] PTT spacebar fails on startup until voice toggled off/on

Repro

1. Enable voice mode (/voice), exit Claude Code
2. Restart → hold spacebar → no speech detected
3. Toggle voice off then on → spacebar works

Environment

• Windows 10 Pro 10.0.19045
• Consistent across restarts

Root cause hypothesis

Audio pipeline initializes on toggle (off→on) but not when restoring persisted voice state on startup. Full analysis + recommended fix:
