Voice mode (/voice): spacebar PTT stops activating, types spaces instead

Description
When using /voice mode, the spacebar push-to-talk intermittently stops working. Instead of activating voice input, holding space just types spaces into the input. The only workaround is toggling voice mode off and back on.
Steps to reproduce
- Enable
/voicemode - Use spacebar PTT normally — works fine initially
- After some time (variable), holding space starts typing spaces instead of activating voice
- Toggling voice off and on again fixes it temporarily
Environment
- Windows 11 Home 10.0.26200
- Claude Code latest (installed via native)
- Bash shell
- Note: OpenWhispr (separate PTT app using RCtrl+RAlt) may also be running — possible keyboard hook conflict?
Expected behavior
Spacebar should consistently activate push-to-talk in voice mode without needing to toggle the mode.
Actual behavior
Spacebar PTT silently fails and starts inserting space characters instead. No error message shown.
