[BUG] "No, keep planning" triggers follow-up suggestions instead of waiting silently

When the plan mode dialog appears with the three options:

  1. Yes, and auto-accept
  2. Yes, and manually approve edits
  3. No, keep planning

...and the user selects "No, keep planning", Claude immediately responds with follow-up suggestions for what to plan next. It should stop and wait silently for the user to speak first.

Claude stays in plan mode and waits silently for the user's next input. No suggestions, no follow-up prompts. The option says "keep planning" — that means the user is in control of what happens next, not Claude.

  2. Give Claude a task and let it generate a plan
  3. When the "Accept this plan?" dialog appears, select "3 — No, keep planning"
  4. Observe that Claude immediately responds with suggestions instead of waiting
