Voice mode: "Voice stream error: Connection closed" immediately on spacebar hold

Description

Voice mode (/voice) consistently fails with Voice stream error: Connection clos… (truncated in UI) immediately when holding spacebar to record. The connection drops as soon as recording begins — transcription never completes. This is not intermittent; it has never worked.

Steps to Reproduce

1. Enable voice mode with /voice → shows "Voice mode enabled. Hold Space to record."
2. Hold spacebar to begin recording
3. Connection immediately drops with error

Attempted Workarounds (none worked)

• Toggling voice off/on multiple times (/voice to disable, /voice to re-enable)
• Fully restarting Claude Code and re-enabling voice mode
• Multiple sessions / restarts

Observed Behavior

• Voice mode enables successfully without error
• As soon as spacebar is held to record, the stream errors out immediately
• Error displayed in red: Voice stream error: Connection clos…
• Has never successfully transcribed — fails 100% of the time

Expected Behavior

Holding spacebar should record audio and transcribe speech to text.
