[BUG] Voice stream error: Connection closed: code 1006 (macOS)

Description

Voice mode enables successfully ("Hold Space to record") but when attempting to record, the following error appears:

Voice stream error: Connection closed: code 1006

Environment

• Claude Code version: 2.1.69
• macOS version: 26.3 (Tahoe), Build 25D125
• Terminal: macOS Terminal.app
• Microphone permissions: Granted to Terminal

Steps to Reproduce

1. Run /voice to enable voice mode
2. Hold Space to record
3. Error appears: Voice stream error: Connection closed: code 1006

What I've Tried

• Restarted Claude Code
• Updated to latest version (2.1.69)
• Granted microphone permissions to Terminal (was not previously listed in Privacy & Security > Microphone)
• Verified network connection is stable

Expected Behaviour

Voice recording should work after holding Space.

Actual Behaviour

WebSocket connection closes immediately with code 1006 (abnormal closure).
