Bug: VSCode extension tab gets stuck on "Thinking" after restart

What's Wrong?
After closing and reopening VSCode, the previously open Claude Code tab
gets stuck showing "Thinking" indefinitely and never responds.
Previously this worked fine - conversations would persist across restarts.
Now I have to close the stuck tab and open a new one every time.
- OS: Windows 11 Pro
- Model: Opus 4.6
- VSCode Extension (not CLI)
- Thinking mode: off
- No "Past Conversations" or "Resume" option visible in the menu
