[BUG] It's hanging again in vscode as of 2.1.65

This is almost identical to the above error which comes an goes, now it's worse. Clause continues to work even though the UI doesn't update. So it's still disconnected from the UI and you can't use it, but now it's actually running.

What Should Happen?

The UI should be connected to what claude is doing.

Error Messages/Logs

There is no log, when you re-open the thread you can see responses and error saying that it couldn't get tool approvals.

Steps to Reproduce

Run VSCode
Start a chat inside of the claude extension.
Usually once a message has been sent it was reliable, but now it doesn't matter and it doesn't catch itself.
The claude cli works fine.

This keeps on coming an going every few versions since like 2.1.40. I had to open and close the issue several time and now it's even more broken. Please repair.
