[BUG] Resume picker does not show custom session name set via /rename

What's Wrong?

Resume picker does not show custom session name set via /rename

What Should Happen?

shows first message content / slug for most of the previous sessions in /resume command dropdown

Steps to Reproduce

1. Start a session
2. /rename my_session_name
3. Exit, then run claude --resume
4. The picker shows the first message or auto-generated slug, not "my_session_name"
