[BUG] /resume still not showing custom session names set via Ctrl+R rename

What's Wrong?

Sessions renamed via Ctrl+R in the /resume picker inconsistently display their custom names. Some renamed sessions revert to showing the first user message as the title, while others (e.g., "convo audit") correctly display the custom name.

This bug has been reported multiple times (#24065, #28269, #29425, #32022) but all were closed as stale/duplicate without a fix. The issue persists as of the latest version.

What Should Happen?

All sessions renamed via Ctrl+R in the /resume picker should persistently display their custom name.

Steps to Reproduce

  1. Start multiple Claude Code sessions
  2. Use /resume, navigate to a session, press Ctrl+R to rename it
  3. Exit and re-open /resume
  4. Some renamed sessions show the custom name, others revert to the first message text
