[Bug] Claude ignores user instructions and operates independently despite claude.md configuration

Bug Description
Claude keeps working on its own agenda. Doesn’t respond to what I say. Doesn’t follow my instructions as outlined in claude.md file. Opus 4.5 never had this issue.
