[Bug] Claude Code ignores user guidance in memory/rules and repeats mistakes independently

Bug Description
see how absurd it is about making dumb mistakes and assumption and how often it repeats the same mistakes and even when i write them in global memory or rules or guidance it doesnt matter. it operates almost independently of everything sometimes. not a bad thing to think beyond what the user's limited context and consideration, but then bring it up as dvice for consideration first
