[Bug] Claude ignores user feedback and repeats corrected mistakes in subsequent attempts

Bug Description
had multiple times today where i gave feedback about what it was doing wrong and then it continued to make the same mistakes. See above convo where i kept calling out that it missed creating a list of apps and then still made it on final pass. happened in another chat as well
