[BUG] Claude Code bypasses Plan Mode and operates under "Ask before edits" Mode

What's Wrong?

When I ask Claude code to suggest or implement a bug fix, a code change, a new feature, etc. it will behave as though it is in the "Ask before edits" mode even though I have explicitly set it to "Plan Mode". I have experienced this issue across multiple sessions for the past few days now.

What Should Happen?

When I switch the extension to "Plan Mode" it should always produce a plan that I can either accept or reject.

Steps to Reproduce

  1. Install VSCode
  2. Install Claude Code VS Code extension
  3. Open a chat in the extension menu
  4. Enter plan mode in the chat and ask it to build something or make a change. It can be hit-or-miss but usually after accepting a plan then switching back to plan mode then asking it to fix a bug caused by the previous plan will result in the extension behaving as though it's in "Ask before edits" mode when you've switched it to plan mode.
