[Bug] Planning mode questions not displayed in question tab, only visible on answer submission page

Bug Description
In planning mode claude asks me the questions. However, I don't see the question itself. I see the "Theme" of the question in the "tab" on the top, the answer options, but not the question itself. On the "page" of submitting my answers I see the questions. This problem exists for a couple of weeks already for me.
