[Feature Request] Implement context persistence and mid-generation code review with inline comments

Bug Description
slow, every request takes a long time; agent seems to keep forgetting things, I need to keep reminding. it strays very quickly if not and code becomes unusable. the only way to make this productive on a medium+ code-base, i have to review every line of code, cannot put it in auto-accept. it would be good if there's a way to select code lines in an IDE and add comments like we do with CRs, but as the code is being generated.
