[FEATURE] Allow renaming conversations in the VCS panel

Problem Statement

When I open VSC in my project, and look at the Claude pane when there is no conversation, l struggle to identify the correct conversation because the auto generated titles are useless for this.

Proposed Solution

Right-click on conversation → "Rename" option, or click-to-edit the title

Alternative Solutions

Search by content to find the correct conversation.
Try to remember in the future to begin conversations with a good title
