Allow renaming Claude Code sessions in VS Code

Description
Allow users to rename Claude Code sessions in the VS Code extension. Currently, sessions are auto-named based on conversation content and there's no way to customize the name.

Use Case
When working on multiple tasks across sessions, it's hard to identify which session corresponds to which task. Being able to give sessions meaningful names (e.g., "Debug auth flow", "Refactor DB layer") would make it much easier to navigate between them.

Suggested Behavior

• Right-click or double-click a session name to rename it
• Or add a "Rename Session" option in the session menu / command palette
