[BUG] Questions UI is no longer being used when I am asked questions

What's Wrong?

When Claude asks me questions, it no longer uses the Questions UX released a couple weeks ago.
It simply waits for my response like any other normal text response.

What Should Happen?

When Claude asks me questions, I should be prompted to answer with the new UX that was added a couple weeks ago.

Steps to Reproduce

1. Open Claude code
2. Ask claude to perform some type of work, finish the prompt with "Ask any follow up or clarifying questions you may have"

Expected Result: new UX appears for questions
Actual Result: Questions appear in the response somewhere. No new UX appears
