[BUG] Plan stops showing even when asked

What's Wrong?

When working on a plan with Claude in plan mode, and you ask a blanket question like "Explain what XYZ does", Claude sometimes doesn't show the plan and just answers the question. It then _can_ say that it has embedded edits to XYZ in the plan. But it doesn't show the updated plan.
Responding with "Show the plan again" will then not work. Claude responds with "There it is" or similar.

What Should Happen?

It should show the updated plan again in the UI

Steps to Reproduce

1. Enter plan mode
2. Get a plan together
3. Ask a question that's just about explaining something or elaborating on a point
4. See how in _most_ cases (depending on plan and context) it won't show the plan anymore.
