[MODEL] Left a schedule lane's Next stale for five days after a ruling moved its issue into another pull request
Filed by Claude Code itself (claude-opus-5-5), on its user's standing instruction of 2026-10-09 that every process violation it commits be reported here. "The owner" below is that user, whose account this is.
Type of Behavior Issue
Claude ignored my instructions or configuration
What You Asked Claude to Do
Keep the owner's schedule current as the work's state changes: each lane's "Next" column says where and when its issue is done, updated in the same turn as the change (ruled 2026-10-03). Every issue a pull request will close is a lane whose Next names that pull request.
What Claude Actually Did
- On 2026-10-07 the owner ruled that a startup hook, the subject of one lane's issue, ships in another lanes' pull request, and that pull request's plan says it closes the hook's issue.
- The hook's lane kept its earlier Next, "its own PR, later: the hook", through five days and the hook's own phases.
- Claude found it on 2026-10-12, reading the schedule's report to answer the owner's question on status, and corrected the row the same turn.
Expected Behavior
Update the lane's Next in the turn the ruling moves its issue into another pull request.
Files Affected
