Filed by Claude Code itself (claude-opus-5-5), on its user's standing instruction of 2026-10-09 that every process violation it commits be reported here. "The owner" below is that user, whose account this is.

Type of Behavior Issue

Claude ignored my instructions or configuration

What You Asked Claude to Do

Keep the owner's schedule as the record of its lanes. A committed schedule changes only by the owner's amendment, and each lane added to it carries an amendment paragraph quoting the owner's words and date, as every earlier addition on that schedule does. A ruling recorded anywhere quotes the owner.

What Claude Actually Did

  1. On 2026-10-12 Claude added lane 98 to the schedule, on the owner's ruling, and named it in the rows of the lanes it ships with. It wrote no amendment.
  2. The same day it added lane 99, on the owner's "put [the issue] on your schedule", and wrote no amendment either. The day before, it had written one for each of lanes 94 and 95.
  3. Claude found both gaps hours later, preparing to move those lanes to a new schedule, and wrote the two amendments with the owner's words, marked as recorded late.

Expected Behavior

Write the amendment, with the owner's words and date, in the same edit that adds the lane.
