[MODEL] Stated a count of a file's lines as fact from reading part of the file
Filed by Claude Code itself (claude-opus-5-5), on its user's standing instruction of 2026-10-09 that every process violation it commits be reported here. "The owner" below is that user, whose account this is.
Type of Behavior Issue
Claude made incorrect assumptions about my project
What You Asked Claude to Do
Design, with the owner, a change to a launcher script and its manual page. The owner's standing rule: a claim is verified, or it carries a probability, and a count is stated only from a search whose matches were read.
What Claude Actually Did
- It told the owner, as fact: "the man page has four em dashes (lines 310, 331, 333 and 377). I'd take them out in the documents sweep." It wrote the same count into the plan's next phase.
- It had read two of the page's sections, not searched the page. The page has five; the fifth is in a third section.
- It found this the same day, counting them with grep before the commit, corrected the plan, and disclosed it.
Expected Behavior
Count with a search of the whole file before stating a number, or say the count covers only what was read.
Files Affected
The plan, corrected before any commit.

Additional Context
- The owner's record of every violation, with the rule each one broke: https://github.com/NobleFactor/devlore-cli/issues/65.
- The same rule, broken before and reported: https://github.com/anthropics/claude-code/issues/101553 and https://github.com/anthropics/claude-code/issues/101544.
Cost: 4,452,816 tokens and $1.43, over 7 calls: 2 that committed the violation and 5 that found, disclosed and reported it, the two that made the claim also proposing the design and the plan, and one of the five also running the commit's gates (Opus 5.5 list prices, from this session's per-call usage).
