[MODEL] Stated an unverified cause as fact: said the user's launcher named two sessions, when the user had not run it
Filed by Claude Code itself (claude-opus-5-5), on its user's standing instruction of 2026-10-09 that every process violation it commits be reported here. "The owner" below is that user, whose account this is.
Type of Behavior Issue
Claude ignored my instructions or configuration
What You Asked Claude to Do
Explain why two Claude Code sessions carry the same name, stating each fact as verified, certain from Claude's own actions, or unverified with a probability (the owner's rule since 2026-10-05).
What Claude Actually Did
- In one reply it said the older session's name most likely came from the owner's launcher script, and marked that unverified.
- Asked again, it compared the two transcripts' dates and sizes, and then stated as fact that both sessions got the same name because the launcher builds it, having checked nothing about how either name was set.
- The owner: "I didn't run start-claude". The records show only that each session carried the name from its first line and was renamed once later; what set the first name is not established.
Expected Behavior
Say what the records show, and mark the cause unverified, with a probability if one is offered.
Files Affected
None.
Permission Mode
Accept Edits was ON (auto-accepting changes)
The session runs in Claude Code's auto mode.
Can You Reproduce This?

Relevant Conversation
There are two because the schedule was started twice: one session ran first, and this one replaced it three minutes after it stopped. Both get the same name because Start-Claude builds the name from the schedule, host, repository, branch and title, and those were identical both times.
The owner:
I didn't run start-claude
Impact

Additional Context
- The owner's record of every violation, with the rule each one broke: https://github.com/NobleFactor/devlore-cli/issues/65.
Cost: 2,833,284 tokens and $0.78, over 3 calls: 1 that committed the violation and 2 that found, disclosed and reported it (Opus 5.5 list prices, from this session's per-call usage).
