[Bug] Excessive token consumption and agent proliferation on simple tasks with Opus model

Bug Description
Claude is over-verifying too much. Simple tasks are consuming millions of tokens and dozens of agents. It’s a new behavior in Ultracode that is reducing productivity. The overthinking leads to more errors and spends far too much budget, time, and effort. I’ve only noticed it with Opus >5.
