[BUG] VSCode integrated terminal shows warning after Window reload

What's Wrong?

If you have an integrated terminal open and you Developer: Reload Window or close and reopen VSCode manually, this warning will show:

Running on Windows 11 with latest version of VSCode and Claude Code.

What Should Happen?

This warning should not be displayed.

⚠️ The following extensions want to relaunch the terminal to contribute to its environment:
- Claude Code for VS Code

Steps to Reproduce

1. Open an integrated terminal
2. Restart VSCode by:

• running Developer: Reload Window from the command palette
• closing and reopening VSCode manually
