[BUG] VS Code extension: /compact command unresponsive after window reload

Summary

After reloading the VS Code window, the /compact command in the Claude Code extension becomes unresponsive. Typing the command in the chat input does nothing until the extension is reloaded a second time.

Steps to Reproduce

1. Install the Claude Code VS Code extension 1.18.0.
2. Open a workspace and open the Claude Code panel.
3. Reload the VS Code window (Developer: Reload Window).
4. Type /compact in the input box and press Enter.
5. Nothing happens; the input stays and no confirmation is shown.

Expected Behavior

The /compact command runs and the conversation is summarized.

Actual Behavior

The command is silently ignored until the extension is reloaded a second time.
