What's Wrong?
When launching claude from the VS Code integrated terminal, 3 new empty VS Code windows are opened
unexpectedly.
## Environment
- OS: Windows 11 Enterprise (10.0.26100)
- Launched from: VS Code integrated terminal
What Should Happen?
Claude Code should connect to the existing VS Code instance without opening any new windows.

Steps to Reproduce
- Open VS Code
- Open the integrated terminal
- Run
claude - Observe 3 new empty VS Code windows opening
