What's Wrong?

When launching claude from the VS Code integrated terminal, 3 new empty VS Code windows are opened
unexpectedly.

## Environment

  • OS: Windows 11 Enterprise (10.0.26100)
  • Launched from: VS Code integrated terminal

What Should Happen?

Claude Code should connect to the existing VS Code instance without opening any new windows.

Steps to Reproduce

  1. Open VS Code
  2. Open the integrated terminal
  3. Run claude
  4. Observe 3 new empty VS Code windows opening
