[BUG] Model skips a mandated step (read the linked issue) in a written PR review procedure

The user's always-loaded project instructions define a PR review procedure whose step 2 is to read the whole diff plus the linked issue and the full comment thread before any verdict. The PR description referenced a related issue. The model read the diff and comment thread, verified the AWS behaviour, and then issued a verdict without opening the referenced issue. It only read it after the user asked whether it had.
Expected: every step of a loaded, mandatory procedure is executed before the verdict, including following issue references in the PR body.
