Model misses a structure-rule violation it enforced earlier in the same session

What happened
Earlier in the session the model folded an extra test file back into the single per-service test file, enforcing a one-file-per-service rule. In this review it did not flag a change adding five new per-feature test files, nor that the maintainer's request in the thread had not been met.

Expected
Apply project conventions consistently, including ones already enforced in the session.

Context: a project CLAUDE.md plus the user's auto-memory define a mandatory, ordered code-review checklist (read every touched file, the whole diff, linked issue and thread; cite the normative source per component; grep callers; check duplication, new structure and scope; then a single short verdict). The model had the checklist loaded in context and had been reminded of it repeatedly in the same session. Reviewing a multi-thousand-line pull request.
