[FEATURE] Add changelog to VSCode extension

Problem Statement

Instead of having to jump into this repo every time VSCode updates the extension to learn of what is new, let's have the changelog available in the extension view (as other extensions)

Proposed Solution

Ship CHANGELOG.md with the extension, so that VSCode can show the Changelog tab in extension view.
It'd be nice if the changelog would be stripped of parts that are irrelevant to the VSCode extension.
Also, think of the size of the file - maybe ship only a limited number of releases, instead of since "0.2.21" like the CHANGELOG.md in this repo.
