[BUG] Phantom usage after weekly reset: 96% of 5h session + 35% of weekly Fable consumed with zero API requests from any client (Max 5x)

What happened

• Weekly limits reset on Mon 2026-09-21 03:00 MSK. (The previous week was legitimately exhausted by 2026-09-17 — my agents were replying You've hit your weekly limit · resets Sep 21, 3am (Europe/Moscow).)
• At ~10:30 MSK the same morning, Settings → Usage showed:
• Current session: 96% used, resets in 2 h 33 min → the 5-hour window opened at ~08:00 MSK
• Weekly, all models: 23%
• Weekly, Fable: 35%
• I was away from the computer the whole time and nothing of mine was running that could have made requests.

Evidence that no client made any requests

I audited every Claude Code client that holds credentials for this account (1 Mac + 2 Linux servers running 4 idle Telegram-channel agents + 1 VM) by parsing all session transcripts (~/.claude/projects/**/*.jsonl, assistant messages deduplicated by message.id, summing usage):

• 03:00 → 10:26 MSK: 0 API requests on every machine. The agents' last real requests were on 2026-09-15/16; after that they only produced <synthetic> "weekly limit" replies. My Telegram messages to them at 10:22 MSK were only queued.
• The only activity after the reset is the session I opened at 10:26 MSK to investigate this — the meter was already at 96% before its first request.
• Every other LLM-using service on those hosts (a Hermes agent, cron jobs, bots) is configured for non-Anthropic providers (GLM / OpenAI); a scan for sk-ant-* found Anthropic credentials only in the 4 Claude Code agents.

So ~96% of a 5-hour session and 35% of the weekly Fable quota were consumed while no client made requests, and the window "opened" at a time when nothing was running.

Side note

"Log out of all devices" on claude.ai does not revoke Claude Code credentials: after using it, claude in my terminal kept starting and making requests. There should be a documented way to revoke all Claude Code tokens at once.
