Skill: review-plan (built-in)

Current behavior

When review-plan finishes reviewing a plan, its "Executor hand-off" block instructs the next session to run, as its first action:

First action: call mcp__ccd_session_mgmt__set_session_title with "<worktree-slug>-<YYMMDD-HHMM>".

e.g. jazzy-sauteeing-karp-260913-1848, cheerful-sprouting-backus-260913-0759.

<worktree-slug> is the adjective-noun pair auto-generated for the review-plan session's own worktree (review-plan-jazzy-sauteeing-karp-6c947a). It carries no information about what the plan is actually for. The session list ends up full of entries like Review plan jazzy-sauteeing-karp and notice-card-contrast-260913-1925 that are indistinguishable from each other without opening each one.

Why it's set this way (as far as I can tell)

The same slug is reused as the artifact doc_id for the corresponding Plan Tracker row, so the title, the session, and the tracker entry can be correlated by eye. That's a reasonable goal — the problem is only that the shared key is also being used as the visible label, when those are different jobs.

Suggested fix

Keep the slug for correlation, but put the topic first in the visible title:

call mcp__ccd_session_mgmt__set_session_title with "<topic> (<worktree-slug>)"

e.g. Twelve-month weekday calendar (jazzy-sauteeing-karp) instead of jazzy-sauteeing-karp-260913-1848. review-plan already knows the topic — it's what the plan review itself is about — so this doesn't require new information, just reordering what's already in the handoff block.
