BUG: Permission modal does not dismiss on Allow click (Claude Code CLI)

Type: Bug
Priority: High
Status: Open
Reporter: Lukasz Galecki
Assigned To: Unassigned — route to Claude Code GitHub issues
Environment: macOS, iTerm2
Build / Version: Claude Code 2.1.261

---

Description

The tool-call permission modal in Claude Code CLI no longer dismisses when the user clicks Allow. This affects every tool type and completely blocks tool execution. Started immediately after upgrading Claude Code; worked correctly the day before.

Root Cause: Under investigation — correlates with the Claude Code upgrade to 2.1.261, but not confirmed.

Expected Behavior:
Clicking Allow dismisses the permission modal and the requested tool call executes.

Actual Behavior:
Modal remains open after clicking Allow. Click has no visible effect. No error message or log output is shown.

Impact

  • Completely blocks tool execution — no tool call can be approved
  • No workaround available
  • Affects all tool types (Bash, Edit, Write, etc.)

Steps to Reproduce

  1. Start a Claude Code session (v2.1.261) in iTerm2 on macOS
  2. Trigger any tool call that requires permission approval (e.g. Bash, Edit)
  3. When the permission modal appears, click Allow
  4. Observe: modal stays open, click does nothing, no error shown

Happens every time, for every tool type.

Workaround

None identified.

Acceptance Criteria

  • [ ] Clicking Allow dismisses the permission modal and the requested tool executes
  • [ ] Verified across all tool types (Bash, Edit, Write, etc.), not just one
  • [ ] No regression introduced for the Deny path or other modal interactions
  • [ ] Verified on macOS + iTerm2 with the fixed CLI version

Related

None.
