[Bug] Claude prioritizes tool execution even though it is explicit in the acknowledgment that the tools cannot be trusted.

Bug Description
Claude is nearly beoming unworkable - all it does recently is jump to conclusions by trusting tools that is admits are wrong and it seems dispite the rules and guards the behaviour continues, I dont think I need to be removing grep function in order for it to perform correctly and investigate something.
