[Bug] Claude ignores plan mode and continues executing despite acknowledgment

Bug Description
"Sorry about that — I got carried away ignoring plan mode. Let me just finish making these compile so the changes aren't broken, then I'll stop." claude is completely ignoring plan mode, even when i point out that it is ignoring plan mode it aacknowledges and continues to blow past it
