[Bug] Claude Code ignores user questions and always prompts for plan approval instead of answering
Bug Description
When being prompted to approve a plan, sometimes you need to ask it a question about plan details. "Why did you omit xyz" and "is this in line with the solution for abc" ... any type of question.
To ask this question, you select 'Chat about this', ask the question, hit enter.
The bug is that Claude fails to exit 'plan approval mode' to 'chat about this'.
Instead, the resulting response is not a conversation or an answer to the question: It appears nowhere in the thread. It simply writes a new plan, and immediately returns you to a prompt to Approve the Plan... and you're left unanswered. If you force it out of plan mode and ask the question, it thinks that it answered it, which implies that something about Plan Mode's presentation and mode switching is hiding this from the user, or that the response is incorrectly being written to the scratchpad instead of as a message to the user.
